I am aware of the nature of this activity, and hereby assume responsibility for myself and/or child to participate and to be photographed for publicity purposes. I will not hold Spindlebeak, LLC and/or its employees responsible in case of accident or injury as a result of this participation.

Lesson is non-refundable and non-transferable unless a lesson is cancelled due to facility closure or unforeseen circumstances.

Lesson must be paid before the start. Cancellation of a lesson must be done 24 hours in advance.

Spindlebeak, LLC reserves the right to cancel group classes with less than three (3) participants.
